Complete the steps below:

1. Health Forms

All first-year and transfer students are required to submit the Health Examination Form by August 1. This form should be filled out by the student’s primary medical provider within the last year. Other physical forms may be substituted, if all areas of the Northland form are addressed. This form is used by Health Services to monitor what types of health conditions are present on campus and for individual health advice. It is not shared with others, except in cases of emergency.

First-year athletes must also submit Athletes Forms. These form can be completed by the student or parent.
