Services Provided

  • Treatment and assessment of minor illness, injuries, and emergencies
  • Dispensing over-the-counter medications
  • Dressing changes, suture removal, ice packs/hot packs
  • Crutches available on loan

Health Forms

All first-year and transfer students are required to submit the Health Examination Form prior to arrival on campus. This form should be filled out by the student’s primary medical provider within the last year.

Other physical forms may be substituted, if all areas of the Northland form are addressed. This form is used by Health Services to monitor what types of health conditions are present on campus and for individual health advice. It is not shared with others, except in cases of emergency.

First-year athletes must also submit an Athletes Medical History Form. This form can be completed by the student and is recommended to review the form with their medical provider.

Immunization Forms

Northland College is required by the State of Wisconsin to keep records of the immunization status of students living on campus for Meningococcal disease and Hepatitis B, but a complete set of immunization records is preferred.

Please review the Hepatitis B and Meningococcal Disease Information and complete the immunization form.

Students who have not received these vaccines can:

  • receive the vaccine prior to arriving on campus
  • receive the vaccine on campus through Health Services
  • formally waive the vaccine for reasons of medical, personal, or religious conviction. Waivers must be signed by the student (if over 18) and be submitted each year.

Health Insurance

Northland College strongly encourages students to carry health insurance and to be aware of the benefits that are available to them in the Ashland, Wisconsin, area. Students should come to campus with their health insurance card.

Student athletes are required to carry health insurance while in season. Contact your athletics department for more details.

International students are required to carry health insurance and submit proof of same to the Campus Nurse before July 31. Please contact the campus nurse at healthservices@northland.edu for more information.

Northland College belongs to an organization that offers student health insurance plans. Plans are finalized by June for the upcoming school year. Learn more and view insurance details.  For questions about the plans, contact WPS directly at 800-221-5573.  Please note that WPS considers Main Street Clinic (staffed by a medical doctor and a nurse practitioner) in Ashland to be our Student Health Center.
