This form can be used to report incidents of sexual misconduct within the Northland College community. Sexual misconduct includes sexual assault, dating violence, domestic violence, stalking, sexual or gender harassment, or retaliation, pursuant to the Sexual and Gender-based Misconduct Policy.
If this is an emergency, if there is an immediate risk to the person of concern or to others, or if you believe a crime has occurred, please contact Campus Safety at 715-682-1399 or call 911.
All information provided in this form will remain confidential except for cases that require mandatory reporting or if otherwise specified by the reporter. Any Northland College student, faculty, staff, or visitor can submit a report.

Please provide as much information as possible. Submissions may be made anonymously, but please note the College’s ability to respond to an anonymous complaint is limited. If you are a mandatory reporter, you must provide your name.
